Before you start: who signs, and for whom
The company is the grantor. It signs through the directors who have signing power in its company certificate, in the combination the certificate states, for example two directors together with the company seal. Attach that company certificate to the power of attorney.

Step by step
- 1. Use company letterhead showing the company's name, address and telephone number.
- 2. Write the place and date of signing at the top.
- 3. Grantor: the company's full registered name, registration number and head office address, then the names of the authorised directors as they appear in the company certificate.
- 4. Grantee: each person's full name, ID card or passport number and address. If several people may act separately, name them all.
- 5. Tasks: name the agency and the law (for example the Cosmetic Act B.E. 2558), and list the acts: submit applications, amend them, give explanations to officials, receive notices and certificates. Name the products or say "all products of the company" if that is intended.
- 6. Period: a start date and an end date, or "until completion" for a single matter.
- 7. Liability sentence: the company accepts all acts done by the grantee within this authority.
- 8. Signatures: the authorised directors sign and apply the company seal if the certificate requires it; the grantee signs; two witnesses sign.
- 9. Stamp duty: attach the stamp (10 or 30 THB) and have it cancelled by crossing it and writing the date.
- 10. Attachments: company certificate, and copies of the ID cards or passports of the signing directors and the grantee, each signed as a true copy.
Thai FDA online filing: Open ID and system rights
Thai FDA's e-submission system (Skynet) uses a personal Open ID account tied to each filer's ID number, not a company login. The company therefore gives a power of attorney to each person who will file, and the FDA opens system rights for those people under the company.
